The Georgia Department of Public Health Home Visiting (DPHHV) Program connects families with support, education, and community resources during pregnancy and early childhood. Home visiting services are designed to help families have healthy pregnancies, support child development, and build strong foundations for children and caregivers.
Families in the Southeast Health District can connect with local home visiting staff for support and information related to:
- Healthy pregnancies and postpartum care
- Infant and child health
- Parenting support and education
- Child growth and development
- Safe sleep and injury prevention
- Breastfeeding and nutrition support
- Connections to community resources and services
What Type of Services Will DPH Home Visiting Provide?
DPH Home Visiting Program services include: 
- Clinical assessment of mother and baby
- Care coordination
- Case management
- Education for high-risk pregnant women and their families
- Linkages to needed resources
Who Can Participate?
Services may be available for pregnant individuals, parents, caregivers, and families with young children living in Appling, Atkinson, Bacon, Brantley, Bulloch, Candler, Charlton, Clinch, Coffee, Evans, Jeff Davis, Pierce, Tattnall, Toombs, Ware, and Wayne counties.
