Position Title: Communicable Disease Specialist/Linkage Coordinator

Paygrade: H, I, or J Dependent upon Qualifications (Unclassified)


Description of Duties:

Plans, develops, and conducts communicable disease-related investigations within the health district or assigned area. Ensures implementation of timely disease intervention strategies to promote disease control including the interviewing of patients, contact tracing, and notifying partners of exposures.  Links HIV-positive individuals to care and re-engages HIV-positive individuals back into care.  Collects, analyzes and interprets data and disease-related information and prepares epidemiologic reports for programmatic purposes. Provides limited training, technical assistance and consultative services regarding the control of diseases. This is the entry/training level of the Communicable Disease Specialist job series. Incumbents at this level may require additional training or experience to gain full proficiency in some or all of the job responsibilities. Performs various other duties as assigned in support of section and district operations.

Minimum Qualifications:

Bachelor’s degree from an accredited college or university OR Four years of outreach experience.

Preferred Qualifications:

Major or minor coursework in the behavioral or life sciences;  experience working in a public health setting;  knowledge of computer applications related to public health (i.e., Immunization Registry, SendSS, M&M); knowledge of communicable diseases; skill in interviewing/counseling; demonstrated ability to prioritize activities, manage time and complete assignments with minimal supervision; demonstrated ability to work and communicate with people from varied backgrounds on sensitive topics.
