
The purpose of the
Health Check program is to assure that children who are enrolled in
Medicaid or PeachCare for Kids receive regular well-child check ups.
The following services are components of Health Check assessments:

Comprehensive health
and developmental history

Vision and hearing
screening

Immunizations
-
Nutritional assessment
-
Age appropriate developmental
assessment through DENVER II for those aged 0-4 months and psychosocial assessments and school success
assessments for those 5 to 21 years of age

Anticipatory guidance
-
Comprehensive physical
examination
-
Blood pressure checks
-
Tuberculosis risk
assessment and skin tests
-
Lead assessment
Georgia Childhood Lead Poisoning Prevention Program
-
Appropriate lab tests
-
Dental referrals
Eligibility:
Eligibility is determined by the financial status of the family.
All persons from birth to 21 years of age that are enrolled in
Medicaid and birth to 19 years that are enrolled in PeachCare for Kids
are eligible for Health Check services.
Access:
Health Check services are available at all health department
locations in the Southeast Health District through scheduled
appointments.
